How to ENROLL your Afterschool Club Kindergarten Student:
Complete the 2007– 2008 Fall Registration Form* and Payment Form. (1 per child)
Review the Parent Policy Handbook and sign the Policy Handbook Form.
Mail all forms, along with a $100 Deposit per child (applied towards your last week’s tuition) plus a $50 Registration Fee per child to the above address.
CHILD MUST BE 5 YEARS OLD BEFORE SEPTEMBER 1, 2007
First day of school is September 3, 2007.
Last day of school is May 30, 2008.
Enrollment is on a first come firstserve basis.
Copies of the child’s birth certificate, physical and dental examinations are required at the time of registration.
