REGISTRATION FORMS

(Download, complete, and return to ASC)

PLAY DATE PROGRAMS

Download and complete the Fall Registration Form, Payment Form, and Emergency/Medical Form.

Review the Parent Policy Handbookand Guidelines.

Downloadand sign the Handbook and Guidelines Agreement.

Mail all completed forms, along with a $50.00 deposit per child (applied towards your last week's tuition) plus a $40.00 registration fee per child to the address below. (Deduct $10 for Instant Savings if submitting your registration on or before July 2, 2007.)

If your child was enrolled in the Afterschool Club within the last year, you should have received a Pre-Printed Emergency/Medical Form. If you have not received one, please call 847.548.2445 to request a copy.

Please review the form and make any necessary corrections.

Download and complete the Registration Form and Payment Form.

Review the Parent Policy Handbook and Guidelines.

Download and sign the Handbook and Guidelines Agreement.

FALL REGISTRATION DEADLINE IS

AUGUST 10, 2007

Registrations received after the August 10th deadline may be delayed in processing for safety reasons.

Your child will not be allowed to attend until your registration is processed.

No same-day registrations or drop-ins allowed for safety reasons.

Late registrants will be entered on a space-available basis and will be notified of their start dates.

IMPORTANT: If your form is incomplete, unsigned, or is missing a deposit, your child's enrollment will be delayed and may be placed on the waiting list. Please note that Afterschool Club works closely with your school and follows safety enrollment procedures. Therefore, registrations received after the August 10th deadline may be required to be placed on a waiting list for verification. Call 847.548.2445 regarding space availability for your Afterschool Club locations.
