How to ENROLL your child at SchoolHouse Children’s Center:

Complete the following 2007-2008 forms:

Registration Form

Social History/Health Form

Emergency/Medical Form

Payment Form

Review the Parent Policy Handbook (and Addendum to Handbook for Hawthorn School Staff) and sign the Policy Handbook Form.

(You can also download, for your convenience, the Parent Reference Sheet and the

Hawthorn Staff Reference Sheet containing the Tuition Schedule and Tuition Schedule for Hawthorn Staff.)

Mail all forms, along with a $100 Deposit per child (applied towards your last week’s tuition) plus a $50 Registration Fee per child, to the address listed below.

Thank you for your interest in Afterschool Club SchoolHouse Children's Center program for 6 WEEKS - 5 YEARS OF AGE for the 2007 - 2008 school year! At SchoolHouse Children's Center, we take pride in making great memories for our children in a safe and nurturing environment. Throughout the school year your child will develop social skills within his/her age group. Avoid waiting lists and register your child before July 2, 2007, to receive a $10 Instant Savings on your registration fee.
